HOW IT WORKS

If more than one MOTIX batteries were purchased, up to 4 batteries can be registered at once on this form.
Once everything is confirmed, IDX will issue a check for the total amount and will mail it directly to the address provided.
(Make sure to double check your name and address, as we will not reissue the check for any reason.  Please allow up to 4 weeks to receive the check in the mail.)
